HEALTH & MEDICAL BENEFITS
As an employee of St. Anthony Hospital, you will have the flexibility of designing your own personal benefits package. FlexCare offers you the opportunity to pick and choose the medical, dental, vision, life, accidental death & dismemberment, dependent term life, long term disability, short term disability, supplement life and long term care insurance options that will meet your individual needs. In addition, both health care and dependent care spending accounts are available on a voluntary basis. All premiums for the Flexcare benefit plan are through pre-tax payroll deductions!
Eligibility
Full-time and part-time employees can enroll in the FlexCare benefit plan within 30 days of employment. The effective date of the coverage will begin on the first of the month following that first 30 days of employment. (For example: If your Employment Date is January 15th, you will have 30 days or until February 15th to turn in insurance elections and then your insurance would be effective on March 1st.)

You may also cover an adult (who may not be your spouse) in addition to yourself on our medical, dental, and/or vision insurance plans known as a "legally domiciled adult." Changes may be made annually to your medical insurance coverage at open enrollment. A prescription drug program is included in all options listed above and includes a co-pay of $6.00-$20.00, based on the prescription. In addition, employees who do not use tobacco products and elect one of SSM Health Care’s medical plans will receive a tobacco-free taxable maximum incentive of $60 annually. This incentive will be paid in two increments of $30 on each FlexCare Free pay period.
Dental Insurance Options
Two dental options are offered with different deductions, maximum out-of-pocket limits, and orthodontic coverage. The two options pay different percentages of basic and restorative services. Orthodontic services are not covered under the low option. You are locked into your dental insurance option for two years.

Vision Insurance Plan
St. Anthony Hospital offers two levels of coverage through Vision Service Plan (VSP) covering examinations or examinations/materials. It provides for one routine exam annually from date of service with a $10.00 co-pay, and a $25.00 co-pay toward prescription frames (every two years) and lenses (annually). The plan provides 100% coverage on contact lenses. You are locked into your vision insurance option for two years.
Life Insurance/AD&D
As a full-time employee, you will receive a life insurance and accidental death & dismemberment policy valued at one times your annual base salary for each policy at no cost to you. You may purchase additional coverage up to four times your annual base salary for an additional charge. Also, you can elect dependent term life for your spouse and all children in the home under 19 years of age. For additional life insurance coverage of up to three times your annual base salary and life insurance policies on your spouse or children, a supplemental package is available for an additional charge through an after-tax payroll deduction from Paragon Life. Some underwriting may be necessary depending on amount. A living benefit and an HIV rider are also available under the policy.
Long Term Disability (LTD)/Short Term Disability
Long Term Disability insurance can provide you with important income protection should you become disabled due to sickness or injury. Full-time employees receive a policy of 60% coverage paid for by SSM Health Care of Oklahoma.
Short Term Disability insurance can provide you with important income protection should you become disabled due to sickness or injury for up to 11 weeks. This is a voluntary employee self-pay benefit designed to assist while waiting for long term disability.
Long Term Care (CNA)
This coverage is designed to protect the employee's and family member's life savings and standard of living should the need for long term care arise. The plan offers employees, employee's spouse, parents, parents-in-law, grandparents, and grandparents-in-law the opportunity to apply for coverage.
Spending Accounts
If you are eligible to participate in the FlexCare medical option, you are also eligible to participate in either or both of the spending accounts, regardless of your medical elections. These accounts (both health care and dependent care) allow you to set aside tax-free dollars to reimburse yourself for eligible expenses. Participation in spending accounts is optional. You may deposit up to $3000 in the health care account and up to $5000 in the dependent care account. Caution: IRS regulations require that any monies in the accounts unused at the end of the year are forfeited into a pool of administrative fees. Participation in the plans would need to be based upon actual expenditures.
Coverage OPT OUT Credits
Employees also have the opportunity to "OPT OUT" of medical and dental coverage under the FlexCare benefit program. Employees who OPT OUT receive a special "OPT OUT BENEFITS CREDIT." These special credits can be applied toward other coverage options, received as an after-tax cash payment, or deposited into a FlexCare Spending Account.
Retirement Benefit
A Defined Benefit Retirement Plan, entirely paid by St. Anthony Hospital, is automatically in effect after one year of at least 1,000 hours of service for employees age 21 and older. You are 100% vested in the plan after 5 years of 1,000 hours or more of service in each year. An early retirement benefit is available at age 55 with 50% of your benefit paid to you. You can start receiving your retirement at age 60 and continue to work for the Hospital. Normal retirement is considered at age 65 with 100% of your benefit to be paid to you.
Tax Deferred Annuity (TDA OR TSA)
Employees have an opportunity to participate in an SSM Health Care Match Savings Plan. This program is available to all employees. A TDA allows employees to postpone receipt of a portion of salary and receive its value upon retirement. We match 50¢ on the dollar up to 3% of your compensation for a maximum employer contribution of 1.5% of your salary up to a legal maximum. Employer contributions are vested after three years. Payments are made through a salary reduction agreement, and are pre-tax contributions. You pay tax at the time actual benefits are received. There are various investment options for you to choose from.
ADDITIONAL BENEFITS
Paid Time Off (PTO)
St. Anthony Hospital provides paid time off for vacation, holidays, short-term illnesses and other personal reasons. The hours accumulate at the same rate for both full-time and part-time employees. Hours begin accruing on the first day you begin employment and can be used after you've been with the Hospital for 90 days. The rate of accumulation increases as your years of service increase.
Extended Medical Benefit (EMB)
The Extended Medical Benefit (EMB) program provides compensation for wages lost when employees take extended time off due to a serious illness or injury during employment. Extended time off is anytime beyond 5 days.
Direct Deposit
All employees may direct deposit their payroll check in the financial institutions they specify on the Direct Deposit Agreement form available in Human Resources. This agreement may be revised anytime by submitting another Direct Deposit Agreement form to the Human Resources office.
Cafeteria
The cafeteria is located on the second floor, main level of the Hospital. Employees receive a discount on their meals when they are wearing their Hospital ID Badges.
Credit Union
The St. Anthony Hospital Employees Federal Credit Union (S.A.H.E.) is located on the 3rd floor of the main Hospital. The Credit Union is an employee-owned organization and is fully insured. Employees may start saving through the Credit Union at anytime with no restrictions on the amount deposited. Members are eligible for loan privileges after thirteen (13) months of consecutive full or part-time status. Checking Account services are also available.
Educational Assistance
St. Anthony recognizes that continuing education is not only a benefit to the employee, but also the Hospital. For this reason, assistance in the form of tuition reimbursement is available to employees for further education in their fields or a related health care area. There are two different plans available, and in some cases, you are eligible upon hire.
Employee Assistance Program (EAP)
The Employee Assistance Program is maintained by the Hospital to provide employees and their immediate families with confidential assistance, guidance and pre-treatment referral for non-medical, personal problems that disrupt job, family and community life. You are eligible for this benefit on your date of hire.
SCORE
This health fitness and medical exam program is located at the St. Anthony North facility. The program assesses your current level of health, and then helps you develop a personal plan for improving your health. Services include: a comprehensive examination, personal fitness assessment and nutrition analysis. This program offers an employee discount on the remaining balance following insurance payment.
Hospital Discounts
Employees and their dependents that elect company sponsored health insurance plans and utilize St. Anthony outpatient services and inpatient services will receive a discount on their billing, Including a prompt pay discount of 25% due on hospital services as long as they initiate prompt payment arrangements within 60 days of insurance payment. Employees will receive a 50% discount off of the cost of services rendered. Please note: Co-pays associated with doctor's office visits, specialty service visits, and emergency room visits are not eligible for discounts; in addition, amounts considered above reasonable and customary by our indemnity insurance plan are not discounted.
Other Discounts
Employees receive a 10% discount on most purchases made in the Hospital Gift Shop.
