Find new ways to discover a healthier, happier you with the SCORE Program at SSM Health Medical Group. Through SCORE, you will receive a complete medical exam and comprehensive fitness evaluation.

The medical exam includes:

• Complete blood and urine analysis
• Blood pressure evaluation
• Lung capacity and function testing
• Exercise tolerance (stress) test
• Screening for heart disease
• Screening for diabetes
• Hearing and vision screening
• Chest X-ray

We perform a fitness evaluation to determine your current fitness level (strength, flexibility, balance and aerobic capacity). Experts will also assess your weight and body fat percentage and create a customized exercise program for you.

If you're a current athlete, you can enroll in the SCORE Athletic Performance Enhancement program, which is designed to help athletes achieve a higher level of performance. In addition to the regular SCORE offerings, as part of this program, athletes will receive a complete blood chemistry profile, sport-specific training recommendations, physician-directed weight management and the option to attend an eight-week weight-loss class.

Following the evaluation, you will receive:

• Specific recommendations by the physician and exercise specialist, including daily plans
• Six-month follow-up visit
• Free Personal Health Record Wallet Card
